In 2009, following the completion of advanced training under the mentorship of Dr. Marsha M. Linehan and Dr. Kathryn E. Korslund, the team formally implemented a standardized DBT program in northern Taiwan. This comprehensive program encompassed individual psychotherapy, group skills training, case management, and regular consultation team meetings. Furthermore, the team actively engaged in research related to DBT, promoted education and training within the Chinese-speaking community, managed a dedicated Facebook page, and contributed to the translation and authorship of DBT textbooks. This presentation aims to delineate the development and organizational strategies of the DBT team at Mackay Memorial Hospital, with a particular focus on sustaining team motivation and fostering continuous growth in the context of managing complex clinical cases. Observed cultural differences and the corresponding adaptations implemented will also be addressed.

Presenter bio:

Shu-Chin Chen is a Clinical Psychologist at the Suicide Prevention Centre of MacKay Memorial Hospital (Chinese Taipei). She is currently a PhD candidate in the Department of Psychology at National Taiwan University and holds a Master’s degree in Psychology from National Chengchi University. She also serves as an adjunct lecturer in the Department of Clinical Psychology at Fu Jen Catholic University for two years. In addition, she was a short-term visiting scholar at the Behavioral Research and Therapy Clinics (BRTC) at the University of Washington.

As a result, accessing neuroaffirmative treatment models can be very challenging, and at times can perpetuate an invalidating environment. This presentation will explore some of the key considerations when working with neurodiverse clients, (including learning styles, sensory processing differences and attentional control) and together we will explore the implications for case conceptualisation, treatment modifications and environmental adaptations that may be helpful.

Cathryn Pilcher is an Occupational Therapist and the Associate Director of Spectrum: specialising in personality disorder and complex trauma, in Melbourne, Australia. Cathryn completed her DBT training with Behavioral Tech, the Portland DBT Institute and other internationally recognised DBT experts. Along with DBT training, Cathryn was integrally involved in developing the Core Competencies Training  for working with BPD and train-the-trainer program as part of the National Training Strategy with the BPD Foundation and has trained numerous clinicians across Australia. In her role she provides training, consultation, supervision and direct treatment and is passionate about supporting systems to work as effectively as possible with people with personality disorder and those presenting with complex needs

Linehan, PhD for multi-diagnostic, severely disordered individuals with pervasive emotion dysregulation. Dialectical philosophy is one of the theoretical pillars of this evidence-based treatment. The theoretical framework underlying DBT – the biosocial theory, reflects the dialectical process. The fundamental dialectic in DBT – acceptance and change, is expressed in the assumptions clinicians abide by in working with clients. Finally, the secondary treatment targets or dialectical dilemmas, are considered in case conceptualization and treatment planning.

Learning objectives:

  • Understand and teach Dialectic Philosophy: What is it and how is it used in DBT
  • Teaching the Biosocial Theory as a transactional model and dialectical process
  • Present the DBT assumptions as reflective of the central dialectic in DBT – acceptance and change

Presenter bio:

Dr. Pamela Buck is a licensed clinical psychologist, Linehan Board Certified DBT Clinician and the Director of Diversity, Equity and Inclusion at the Boston Child Study Center, where she provides evidence-based treatments to young adults and families with mood and anxiety disorders, trauma-related disorders, substance use and eating disorders, attention-related difficulties and personality disorders. She provides CBT, DBT, and evidence-based treatments for PTSD: prolonged exposure (PE), DBT-PE, DBT-PTSD, and Cognitive Processing Therapy.

Dr. Buck provides DBT training in the USA and internationally. She introduced DBT in Vietnam and Malaysia through intensive trainings for psychiatrists and psychologists at the Vietnam National University, Ho Chi Minh City and the National University of Malaysia, Kuala Lumpur.

This presentation will take a deep dive into the components of Chain Analysis and beyond. Briefly exploring the components of chains, the session will focus on the “how to” of effective chain analysis. We will identify common problems and strategies to overcome them, including challenges with engagement, working with “I don’t know”s, and ways to manage severe emotion dysregulation and emotional avoidance while conducting chains. The second half of the presentation will focus on solution analysis, including identification of where to “break the chain” and how to increase client self-efficacy for chain analysis completion between sessions. Dr Sian Jeffery, M Psych (Clinical), Ph.D., is a Senior Clinical Psychologist and Coordinator of the DBT programs at Fremantle Mental Health Service in Western Australia since 2009. Dr Jeffery completed DBT training with Behavioral Tech, the Portland DBT Institute and other international recognised DBT experts. Dr Jeffery was formerly an Australian national trainer for Family Connections® and is an Australian trainer for the National BPD Core Competencies Training strategy. Dr Jeffery has a keen interest in personality disorders and is actively involved in systems change in Western Australia. Dr Jeffery provides regular training in DBT for Psychology Training, as well as supervision and consultation.

DBT skills training is one of the four core components of standard DBT, and has been demonstrated as a key ingredient underpinning DBT’s efficacy in improving clinical outcomes (Linehan et al., 2015). Further, use of DBT skills has been found to mediate the effects of DBT among patients with BPD and other clinical conditions (Karam et al., 2022; Neacsiu et al., 2010), highlighting the importance of teaching DBT skills effectively to maximize clinical benefits. In this workshop, Prof Shian-Ling Keng will present on tips and strategies on starting, structuring, and running a standard DBT skills group in a clinical setting. She will discuss factors to consider when starting a DBT skills group, as well as evidence-based curriculums to reference in developing a DBT skills teaching plan. Tips on skills teaching, conducting homework review, and missing chain analysis will be presented, with an opportunity for interaction with the audience in regards to strategies to run an effective DBT skills group.

Presenter bio:

Shian-Ling Keng, Ph.D. is a Professor in the Department of Psychology, School of Medical and Life Sciences at Sunway University, Malaysia. She is trained as a clinical psychologist, and her research focuses on the effects and mechanisms through which mindfulness-based interventions affect psychological and physical health. Additionally, she is interested in examining cross-cultural presentations of psychological disorders (e.g., borderline personality disorder), and adaptations of evidence-based interventions (e.g., DBT) in low resource contexts.

Dr. Keng received her B.Sc., M.A., and PhD in Clinical Psychology at Duke University in the United States, and completed her clinical residency at Centre for Addiction and Mental Health, Toronto, Canada. Prior to joining Sunway University, she taught at Monash University Malaysia, and was a tenured associate professor and Head of Studies (Psychology) at Yale-NUS College, and assistant professor in the Department of Psychology at the National University of Singapore.

Clinically, Dr. Keng has received intensive training in CBT, DBT, and mindfulness-based intervention approaches. She is a clinician certified by the DBT-Linehan Board of Certification, and a qualified teacher of Mindfulness-based Stress Reduction (MBSR). She serves as Chair of the Provisional Board of the Asia-Pacific Association for DBT, and sits on the Board of Directors of the World DBT Association.
